The Continuous Treatment Team (CTT) provides intensive services to children and youth who are diagnosed as seriously and persistently mentally ill, and identified as at imminent risk of potential hospitalization or other therapeutic out-of-home placement. Program components include care coordination, individual and family therapy, group therapy, and psychiatric services. Comprehensive services include therapeutic intervention, skills building, medication management, school intervention, crisis intervention and stabilization, advocacy, and referral assistance. The goals of the program include symptom reduction, restoration of functioning, reduction of inpatient placements, and maintaining and improving the quality of life for these young people. Many services are delivered in the home or other natural setting.
