Program for Assertive Community Treatment (PACT)

The Program for Assertive Community Treatment (PACT) is designed to help adults with severe mental illnesses reduce or eliminate their symptoms, function in the community, live independently and reduce hospitalization. The program provides intensive, direct care and services are available 24 hours a day, seven days a week to these clients. Services are delivered primarily in the home and in the community, which enhances accessibility and the comfort level of clients. 

PACT offers case management service, medication management services, crisis intervention, advocacy, group therapy, and goal-oriented therapy services. Depending on the individual’s needs, PACT has the capacity to see clients daily. Individuals active in PACT attend medication appointments on a monthly basis and have available to them on-call crisis services 24 hours a day, 7 days a week.
